IBM and Emdeon have entered into a licensing agreement for IBM Initiate Patient that will allow Emdeon the ability to assemble a virtual view of patient data for use by healthcare providers. This virtual patient view will be populated with data from Emdeon’s clinical information network that today processes over 150 million clinical and ePrescribing information exchanges per year. Attend this session and learn how authorized healthcare providers will be able to retrieve a patient’s medical history or other clinical information through a single query of Emdeon’s vast data resources.

The AAFP Physician Metrics and Analytics pilot is designed to be the first such program developed to drive quality improvement while yielding practical insights to AAFP members through an aggregated clinical data repository (CDR). The CDR will be populated with Emdeon medical, pharmacy and lab data from participating providers, and Emdeon analytics tools will deliver practice-specific and comparative benchmarking on a variety of quality and cost measures. The program will allow AAFP to continue its leadership around proof-of-value, patient centered medical home, pay-for-performance and other quality improvement initiatives that are data dependent.

As healthcare reform begins to take hold in the market, Independent Physician Associations (IPA) have an opportunity to grow into the ACO model. Learn how one such IPA (MPS) is working with Emdeon to create the informatics infrastructure necessary to declare itself a clinically integrated group… one of the key steps on the road to being a full-blown ACO. By combining MPS medical and pharmacy claims data in near real-time, Emdeon will be able to provide MPS a foundation for population health reporting, disease registries and other tools that drive clinical quality improvement. From there, MPS physicians will be able to drill down to the individual patient level, and improve health outcomes one patient at a time.
